  • Identify and explain risk factors for type 2 diabetes
  • Identify people at risk of type 2 diabetes and offer them support including referral to appropriate prevention programmes
  • Understand how social determinants of health impact type 2 diabetes risk
  • Understand the importance of non-stigmatising language when supporting people at risk of type 2 diabetes
  • Know how to support someone to reduce their risk of type 2 diabetes and recognise the barriers they may face in accessing the right support
  • Describe current referral pathways and understand how to access local prevention programmes and services.
